Accident report form – what to do after a road accident in the Netherlands

It’s said that every driver experiences at least one minor crash or road accident in their lifetime. What should you do in such a stressful situation, especially in the Netherlands, to ensure everything is presented correctly to insurers and to avoid fraud? Learn how to complete an accident report form!

How to act after a road accident in the Netherlands

  1. Ensure safety first – yours and your passengers’. If there are injuries, try to provide first aid and call the ambulance and police. If the accident has not caused injuries and the cars are still drivable, you don’t need to call emergency services. In such cases, the first step is to move the vehicles to the roadside or a parking area to keep traffic flowing.
  2. Take photos or even record a video of the accident – these often serve as the only evidence of the incident. After that, fill out the accident report form.
  3. Send the completed form to Polisa.nl at info@polisa.nl, along with photos and videos of the incident.
  4. Be wary of fraudsters who may offer to help you fill out the form but could intentionally misrepresent the facts to your disadvantage. Properly fill out the accident report form

Ensure the following information is included:

  1. The other driver’s phone number – check it works by calling it.
  2. Driver’s license details – category, license number, and issuing authority.
  3. Insurance details – name of the insurer, policy number, and coverage period.
  4. Reason for the collision – Mark it in the appropriate section. Important! If you make a mistake, replace the form or cross out the error, sign next to the correction, and have both parties initial the change.
  5. Description of the circumstances – including date, time, location, cause, and a clear identification of who is at fault. Be precise with language (e.g., “sudden braking” vs. “emergency braking” are entirely different scenarios).
  6. A sketch of the accident – Clearly indicate which vehicle is A and which is B.

Once the document is signed by both parties, each retains one copy – either the original or a duplicate. Changes made to the document after signing will not be valid unless signed by both parties.
